We require at least 24 hours' notice for appointment cancellations. Late cancellations or no-shows may be subject to a fee. We reserve the right to dismiss patients who repeatedly fail to adhere to scheduling policies.
Patients are responsible for all co-pays, deductibles, and non-covered services. While we assist with insurance navigation, ultimate financial responsibility rests with the patient. We accept most major insurance plans; please verify prior to your visit.
